I'm surprised at the drop in the number of US investigators. The Bible of medical practice, otherwise known as Medical Economics periodically runs articles on how doctors can pad their incomes by doing trial work on the side. The rule of thumb is that if you put in one our a day seeing trials patients you can jack your yearly income by about 20%. Given that doctors far outranked the other top 20 profession by income in one recent money magazine survey, maybe they don't need the money.
